Family Private Care provides free in-home client needs assessments to individuals who are likely to meet criteria for private home care services. The in-home assessment is performed by a registered nurse, takes one to two hours and includes:
- Current medical information review
- Psycho-social assessment
- Functional/home safety assessment
- Dietary and nutritional assessment
- Complete physical (body system) assessment
- Vital signs
- Head and neck
- Eyes
- Ears
- Nose and throat
- Respiratory
- Neurological
- Cardiovascular
- Peripheral circulation
- Musculoskeletal
- Gastrointestinal
- Genital/urinary
- Skin (integumentary system) assessment
- Activities of Daily Living (ADL) assessment
If the nurse finds that you meet the criteria for private home care services, the nurse will begin the process for admission. If the nurse determines that you meet the Medicare criteria, several agency names will be given.
